MicroNet Mesh-Covered Protection Stent for Carotid Artery Disease: Ten-Year Single-Center Experience.

Abstract

<h4>Background</h4>This study evaluates the early and midterm safety outcomes of CGuard carotid artery stenting (CAS) for asymptomatic carotid artery disease, with a focus on the accuracy of intraoperative stent release.<h4>Methods</h4>We conducted a single-center retrospective study of patients undergoing CAS for asymptomatic carotid stenosis >70% (2015-2025) in a high-volume vascular. The primary end point was the perioperative rate of any death, stroke, or myocardial infarction. Secondary end points included technical success, the necessity for unplanned adjunctive carotid stenting, and operation time as indicators of the ease and accuracy of CGuard stent deployment. Freedom from restenosis and late neurological outcomes were also evaluated. Postoperative evaluations ensured alignment with preoperative planning, verifying stent placement and appropriate lesion coverage. Kaplan-Meier survival estimates of freedom from restenosis were assessed.<h4>Results</h4>One hundred ten patients were included (male 73%). The 30-day clinical success rate was 100%, with no perioperative deaths, myocardial infarctions, or strokes. One patient experienced a transient ischemic attack. The mean operation time was 29 ± 8 minutes, and 4 patients (4%) required additional unplanned stenting. The median follow-up was 37 months, with an overall survival rate of 89% at 60 months. One case of stent stenosis occurred at 22 months, with a 60-month freedom from restenosis rate of 98% and primary-assisted patency of 100%.<h4>Conclusion</h4>CAS with the CGuard dual-layer stent was associated with high technical success, a low rate of early neurological event, and durable midterm patency in asymptomatic patients.
