Abstract

<h4>Introduction and importance</h4>High-voltage electrical burns to the scalp often result in significant defects in soft tissue and calvarial bone. The use of local flaps presents a straightforward surgical approach that can effectively cover exposed skull defects. However, the application of split-thickness skin grafts at the donor site often results in a hair-bearing scalp lacking hair, which adversely affects aesthetic outcomes and diminishes self-confidence in social contexts.<h4>Case presentation</h4>This report outlines a clinical case in which the utilization of a tissue expander was optimized to achieve superior surgical results with minimal interventions. Furthermore, we explore aspects of skull reconstruction and appropriate soft tissue coverage. Technical considerations relevant to the implementation of expanded flaps for managing extensive scalp defects will also be discussed.<h4>Clinical discussion</h4>Scalp tissue expansion is recognized as the most effective reconstructive strategy for addressing hair-bearing defects. It is essential to develop a meticulous surgical plan that may include repositioning the flap, utilizing a skin expander to remedy the secondary scalp defect, and reconstructing the skull defect. Such a comprehensive methodology is instrumental in mitigating the potential complications associated with scalp defect reconstruction.<h4>Conclusion</h4>Scalp reconstruction requires a careful and detailed plan to achieve the best functional and aesthetic results. The scalp tissue expander serves as a powerful tool to obtain the most suitable material, even during secondary procedures.
